Voicing the needs of older people

What We Do

We ensure that older people have opportunity to access the support of an independent person who works with them to ensure that their own desires and interests are taken into account.

We enable older people to make informed decisions about any choices they have to make and to communicate their views effectively.

We provide advocacy and support in the following areas:

• discharge from hospital

• when accessing community care and community care services

• when changes are about to happen in the management or running of a person's care plan

• at reviews in residential or nursing homes

• at assessments in hospital

• at reviews in the community

• when individuals are faced with possible changes in their lives

We provide information and/or referrals on:

• community care policy

• health policy

• housing

• residential and nursing homes
